Cavogram shows unrecognized Simon nitinol filter (Bard, Covington, GA) misplacement in right common iliac vein (arrow) with hooks overlying sacrum. Patient subsequently became hemodynamically unstable, and CT scan showed large retroperitoneal hematoma and filter misplacement in right iliac vein.
