Note medial infiltration into jugular tubercle and occipital condyle with replacement of normal hyperintense marrow (solid arrow), spread into posterior fossa and into internal auditory canal (open arrow), lateral spread into middle ear cavity, and inferior involvement of nasopharyngeal carotid space.
