The results of hydrocortisone-oil retention enema therapy in 16 children with ulcerative colitis are presented with particular attention to the roentgenographic evidence of response to topical steroids. Ulcerative colitis in 8 of the 16 children reversed completely, both clinically and roentgenographically, after daily therapy with steroid enemas. Ulceration, mucosal edema, loss of haustra, pseudopolyps, narrowing, shortening, and abscess formation, were all reversed after such therapy. This study suggests that when oral sulfonamide, oral steroids, and other measures fail to control ulcerative colitis in children, the next step in management should be daily steroid retention enemas. This form of treatment should be continued from months to years.
