A case of traumatic aneurysm of the middle cerebral artery in a patient who suffered a closed head injury with a linear skull fracture is reported. Repeat angiography 7 days after injury disclosed the aneurysm associated with severe vasospasm and intracerebral hematoma. The parent artery appeared to be normal and neither vasospasm nor displacement of the cerebral vessels was present on the previous angiogram obtained 3 days earlier. The aneurysm in this case probably was caused by injury of the arterial wall by the firm sphenoid ridge.
