LARGE VOLUME TRANSLUMBAR AORTOGRAPHY IN AORTIC OCCLUSION

M. LEA THOMAS and E. W. L. FLETCHER

Thirty-six patients with total occlusion of the lower abdominal aorta have been investigated by translumbar aortography using 60 ml. of contrast medium.

In 78 per cent of the patients the full extent of the occlusion was demonstrated and in 61 per cent of the limbs investigated the arterial tree was shown at least as far as the bifurcation of the popliteal artery.

A translumbar aortogram needle with a blocked end and side hole is recommended to minimize stripping of the aortic wall and hemorrhage.

No complications due to the contrast medium, stripping or bleeding were encountered.

The importance to the surgeon, both for treatment and prognosis, in knowing not only the full extent of the aortoiliac occlusion but also the state of the distal arteries is emphasized.
