"Calyceal crescents" are the distorted collecting tubules filled with concentrated contrast medium during the early phase of urography in hydronephrosis due to obstruction.
Following adequate drainage of the collecting system, return to normal collecting tubular alignment may be documented.
The recognition of "calyceal crescents" is helpful in diagnosing hydronephrosis secondary to obstruction (functional or mechanical). The differential diagnosis is discussed.
A few pressure measurements before surgery and urography, following drainage of the collecting system, strongly suggest that for occurrence of the "crescent" phenomenon the resting intrapelvic pressure must be increased.
