Selective oblique pulmonary angiography was performed in 34 patients with a history suggestive of pulmonary embolism following an inconclusive central pulmonary angiogram in the anteroposterior position.
The site for selective injection corresponded to abnormalities on the initial angiogram or perfusion lung scan.
In 28 (82 per cent) of the cases a definitive diagnosis was established by selective catheter technique with oblique positioning.
The technique is simple and there was no morbidity associated with the additional study.
Selective angiography with oblique positioning is recommended for all cases of pulmonary embolism in which the central anteroposterior pulmonary angiogram is nondiagnostic.
