The numerous contacts of the left lung with the mediastinum and its contents have characteristic roentgenologic counterparts which must be recognized to avoid error of over- or under-diagnosis.
In particular, the interface between the left mediastinum and the lung anterior to the pulmonary artery and the aortic arch, when roentgenologically present, is projected medially to the aortic "knuckle," or tangent thereto. When this interface is lateral to the "knuckle," the possibility of excessive geometric distortion must be considered before the suspicion of an abnormal mediastinum is raised.
