Two cases of sole arterial supply to normal basilar segments of the left lower lobe via an aberrant systemic vessel are described. A spectrum of congenital pulmonary abnormalities with systemic arterial supply to the lung is presented. Complete roentgenologic evaluation is necessary prior to definitive surgery.
