This investigation establishes the usefulness of the hilar height ratio concept and reviews those disease processes that may become manifest by an abnormal left hilar height ratio. In the absence of lower lobe volume loss and extrapulmonary disease, an abnormal left hilar height ratio may reflect upper lobe emphysema.
