In working with radiology residents, I have long used my own folksy term for the alliterative error eloquently described by Dr. Berlin [1] in the April issue. I call it the "emperor's new clothes" error, and I encourage the residents to use the simplest of preventives: Read the old reports only after evaluating the images and formulating your own differential diagnosis.
I offer this suggestion as an addition to Dr. Berlin's excellent risk management pointers.
